I, the above named individual, acknowledge that I have voluntarily enrolled in this program and/or class and assume responsibility for all risks of injuries resulting from my participation in activities of Dance and Bmore Inc and/or Dance and Bmore Programs. I certify that I am physically and mentally capable of participating in these activities. I voluntarily agree to release, discharge, and hold harmless Dance and Bmore Inc and/or Dance and Bmore Programs for any and all claims of liability. Should emergency medical services become necessary for the undersigned participant, the expenses are the sole responsibility of the participant and not that of Dance and Bmore Inc and/or Dance and Bmore Programs.

I hereby assume all risk, waive, release and discharge any and all claims of damages for, illness, personal injury, death, or property loss which I may have, or which may hereafter accrue to me against Dance And Bmore Inc or Dance And Bmore Programs, their directors, officers, employees and agents and their local affiliates, other sponsors, promoters, and their respective directors, officers, employees and agents, arising out of or in connection with my participation in Dance and Bmore Inc and/or Dance and Bmore Programs. I agree that this waiver release and assumption of risks are to be binding on my heirs and assigns.
