Sprig + Plate New Client Form
Thanks for your interest in Sprig + Plate meal prep!  Please read over the following information and fill out the required fields to be added to the weekly meal distribution list. If you have any questions or extra info we need to know, there is a space at the end to enter additional comments.  We look forward to working with you! Ordering:
• Weekly menu consisting of five menu options is sent out weekly on Tuesday mornings via email.  Once added to the list, you will receive an email with a link to complete the menu order form.  Please make sure to read each option in its entirety, as there will be information included pertaining to cost, serving size, ingredients, allergens, preparation method, and additional instructions.

• The order link will stay live until Wednesdays at 10AM, unless otherwise noted.  If you attempt to access the form once orders are closed, you will receive a message stating that the form is no longer accepting responses.

• There is no minimum to order each week, and you are not obligated to order weekly.  You're welcome to place orders when you want food and skip any weeks you don't.

• Menu items can be ordered in the following sizes: small (feeds 2/$40), medium (feeds 4/$55), large (feeds 6/$70), and XL (feeds 8/$85).  Pricing is determined by cost of ingredients and can vary on occasion.  

Payment & Delivery:
• Delivery is available to a limited area (Northside Indianapolis and select suburbs) for a flat fee. Food can be picked up at our storefront (located at 3915 E. 96th Street, Indianapolis 46240) to avoid delivery charge, or if you reside outside the delivery area. All food is ready for delivery or pickup on Mondays. Delivery charges are determined based on location. Please contact Molly at meals@sprigandplate.com to determine applicable charge.

• Invoices are sent on Fridays to the email on file.  Payment is due prior to meal delivery/pickup on Mondays.  You have the option to save your credit card to your file if you prefer for us to run your card automatically for your orders.

• Please complete the form below to be added to the weekly menu list. You will receive a copy of your responses via email upon submission.
