Summer Camp Installment Plan Request
We are thrilled to offer installment plans this year for Destination Dance! 

Pay 50% at sign up and the remaining 50% on May 31, 2024. Sign up by May 10th.

If you would like to use the installment plan option, it is only available upon request via this form. 
DO NOT ENROLL YOURSELF. WE WILL ENROLL YOU. Use one form for each dancer.

1. Make sure you have an account created with your students added. CREATE ACCOUNT HERE
2. Make sure you have a card on file. UPDATE PAYMENT METHOD HERE
3. Fill out this form for each dancer and our Customer Care Team will enroll your dancer and send you confirmation.

First cycle of applications are due 3/15 with award notifications going out 3/29.

We will continue to accept applications after the first cycle deadline with award notifications going out on 4/26, and every two weeks thereafter, until all funds are exhausted.

Maria will enroll you, apply the 50% scholarship contingency coupon, and send you the confirmation along with the total remaining balance (scholarship is not guaranteed until official award notifications sent and accepted).

Please be aware of all policies for drops and transfers.

I understand the 1st payment will be charged today and the second payment will be auto-charged on May 31, 2024. 

Charge back/declines will be charged a fee of $15.

You are responsible for this payment even if you cancel your enrollment as we are a NO REFUND program (unless you pay for the Flexible Refund Fee of $75 at sign up).
