This form is a request for materials for Healing Circles. The Facilitator Books and Participant books are each $10 a piece. You will be able to pick up your books from Jay, or we can mail them to you upon request.

***READ THIS BEFORE PLACING YOUR ORDER***

You will be responsible for ensuring your participants pay for their books before you pick them up. We advise instructing your participants to purchase their books prior to your first healing circle gathering so that you can distribute the books at your first meeting.

Step Three: In the comments, write "For participant workbook" or "For Facilitator Handbook"
Step Four: Fill out giver and payment information.
