CES - Learning Option Change Request Form
Please complete this form or email your child's homeroom teacher by October 5, 2020 if you would like your child's learning option changed.  Families should complete one form for each child needing a change.  New class placements will begin Monday, October 12, 2020.  Camden Elementary School will contact you prior to October 12 about the new class placement and other important information regarding your child's Learning Option change.  Please remember that request will not be honored after October 5.  You will have to wait until the end of the 1st nine weeks (November 9) before requesting another change.  Thank you!
