Please thoroughly review this form before meeting with each legislator! This is where you should record ALL conversations with your state representatives. Any interaction you have with a legislator about our legislation provides important information that will help us strategize about which legislators to target when and how. This form will help us compile this information in one place.

Please complete the form for every contact or interaction you have with a legislator. For example, if you speak to them on the phone and then meet with them virtually a couple days later, complete the form to report back on your phone conversation and your meeting.
