Please fill this out only if you are the legal guardian of the student, and your student attended school last year in the Socorro Consolidated School District. You will be asked for information about living arrangements, transportation, siblings who attend school, and medical information. Some information is optional; other information (medical, contacts, and transportation) is required and must be entered every school year.
