Students:  The purpose of this form is to help the Health Careers Academy determine which students can best benefit from the academy experience.  Please ask two teachers if they are willing to complete this form for you. At least one must be a teacher of English, math, science, or world language. Please send them this link.
A few days later, you should check with each teacher to be sure they have completed the form. Recommendations are due February 5, 2022.
