Before joining the Coalition, prospective members must explain their long term commitment to community health and how they meet the following 5 criteria:

1. Endorsement of Design Principles
2. Technical Expertise
3. Sweat Equity
4. Coordination of Individual Efforts with the Group Effort
5. CHW Inclusion

Applications are reviewed on a quarterly basis. 2024 application deadlines are:

• June 7, 2024
• September 6, 2024
• December 6, 2024
