I hereby authorize the representatives, volunteers, and affiliates with Behold Christian Ministry/Alpine Youth Center to use my child's/children's photo and/or information related to his or her experiences with Alpine Youth Center activities and events. I understand this information may be used in publications, including electronic/online-internet publications and postings. Moreover, you agree that your child's/children's photos and general identifying information may be included in audiovisual presentations, promotional clips, advertising, community presentations and/or letters to area legislators, media outlets, and in other similar ways.

You agree to release Behold Christian Ministry/Alpine Youth center from all liability related to or the result of posting his or her participatory information/photos on Facebook, Snapchat, Twitter, Alpine Community Network, Church/Ministry websites, etc. Photos and/or relevant personal materials involving your child's/children's participation in general will be limited to Behold Christian Ministry-Alpine Youth Center, Youth Venture servers and websites and common social media online outlets.

My consent is freely given as a public service to Behold Christian Ministry-Alpine Youth Center without any expectation of monetary or other forms of payment. I release Behold Christian Ministry-Alpine Youth Center non-profit organization volunteers, Board officers, and Directors, all affiliates including volunteers from any and all liability which may arise from used photos or personal participation information in association with social media outlets, videotapes, and written articles.

I understand that I can revoke this release at any time in writing.
