This form must be completed in order for students to participate in the OHS Band Program. Please provide accurate information for all necessary questions. Along with this form, students and parents will need to physically sign a Band Handbook Signature/Acknowledgement Form (see www.OakleafBands.com for those forms).

PLEASE COMPLETE ONE FORM PER STUDENT (families with multiple students in the program must complete one form per student)

*Information collected from these forms will only be used in the OHS Band Department for band-related issues. No information will be shared with outside parties. All cell phone and email use will be limited to the OHS Band Department.
