Study Buddies & After School Information and Registration
After a successful first semester we are continuing with our Study Buddies e-learning hub/camp program and are also offering After School Activities programs (w/ and w/o pick-up) when schools transition back to in-person learning.. At the end of this form, a method of payment will be required to submit registration with a 2 week NON-REFUNDABLE deposit. You will be able to read through the questions (for informational purposes) prior to submitting. SPACE IS LIMITED as we are experienced a high volume of requests this week. Transportation spots are also limited and first come first serve for afterschool. Lunch Delivery is available through Jason's Deli ($4.97) links provided on the Study Buddies FB page (link provided when registration is complete). If requested, CCS may make NOMAD a meal stop. Weekly Tuition Rates
Study Buddies & After School Tuition will be "averaged' over a 21 week period to help create stability (and longevity) of the program for all involved. ***Explanation of Table above/Example:*** (7 weeks@$120)+(7 weeks@$90)+(7 weeks@$60) = 21 weeks@$90. But, if only enrolled for 14 weeks, (7 weeks @$120)+(7 weeks@$90)=14 weeks @$105. Weeks are counted by the time enrolled (not a specific time frame on a calendar.) There may be additional fees requested for specific materials for art/science/enrichment activities or outside programs or field trips. We thought it better - being a new program - to request the activity/materials fee when we need it/have the activity scheduled rather than blindly collecting it up front. COVID Measures:   DO KIDS NEED TO WEAR MASKS? CURRENT: Masks required in classrooms. (GOV EXEC. ORDER) *When ALLOWED, we will operate as "mask optional" for children enrolled in our programs*  *Masks are ineffective in wet/high humidity environments such as pool decks* *Many of our activities take place in chlorinated water* *We have large garage doors we open as weather to circulate air on deck.*  *Our counselor/facilitators will wear a face mask or shield in the rooms* *The front desk folks will be wearing masks.. LOCKER ROOMS and COMMUNITY AREAS are deep cleaned daily and wiped down during the day. COVID NOTIFICATIONS:  If a family chooses to take a student for a COVID test, we will send an email to everyone saying "someone" had a Fever of X degrees (or significant symptoms) on day/night Y and went to go get tested on day Z. We will update you with contact tracing info and test results when available. We are not going to say who it is specifically.  We will send a second email to the people in the same orbit (kids who were in the same room, activity, van, etc. within 24 hours) letting you know. Again, this is just reporting of the SYMPTOMS. When we know if the test taken was negative, or positive, we will let everyone know and provide the information needed going forward at that point. Please INITIAL THAT YOU HAVE READ AND ACCEPT THESE POLICIES. *
SWIM INSTRUCTION: There is NO ADDITIONAL COST for students to receive weekly swim instruction. However, appropriate swim attire will be required for all aquatic activities. No "beach shorts" will be allowed for boys and no "two piece" or loose fitting swim suits will be allowed girls. Swim shirts will be allowed IF they are tight-fitting.  Appropriate well-fitting googles will be required for all (mask-like goggles or those that cover the nose will be permitted). Suits (jammers for boys, swim team "style" for girls) and goggles are available in the NOMAD Pro-Shop and staff can assist with sizing. KARATE INSTRUCTION: There is NO ADDITIONAL COST for students to attend a weekly Karate class. However, there is a 1 time fee to purchase a Karategi (white karate uniform $40) for After School students (not needed for Study Buddies) if the student doesn't already have one. Students will be eligible to attend optional "belt ceremonies" and will pay any fees related to earning belts directly to MATI Karate. SWIM TEAM: Students who are interested and qualified to participate on the Evolution Aquatics swim team may attend practices with Stroke Intro or MS/HS Prep (finish before 6pm) or Stroke Development or Stroke Performance (finish at 6:45pm) if they register with USA Swimming (one time annual registration $125). There is no additional charge to stay at NOMAD until 6:45pm if participating in a swim team practice. Initial you have read these policies. *

2nd Semester: I understand that to ensure the program will run/be sustainable, I will make at least a 7 week enrollment commitment assuming there is no change in public health/school issues that would prevent my child from attending. Please initial I have read and understand this policy. *

TERMS OF AGREEMENT: NOMAD Aquatics & Fitness and its co-organizers are not responsible for lost or damaged personal property. All scheduled events are subject to change.  I understand that NO fees will be REFUNDED or TRANSFERRED unless a child is unable to participate due to an accident or illness with accompanying PHYSICAN's note.  In case of an emergency, I hereby authorize my child to be treated by Certified Emergency Personnel (i.e. EMT, First Responder, and/or physician).  And additional waiver/release may be required prior to starting the program. PHOTO RELEASE: I hereby give permission for my child to be photographed at NOMAD Aquatics & Fitness.  I understand the photos and quotes can be used for promotional purposes and can be posted on a private FB group for AFTER SCHOOL at NOMAD.  I understand that although my child's photograph may be used for advertising, his or her identity will not be disclosed.  I do not expect compensation and that all photos are property of NOMAD Aquatics & Fitness and its affiliates.   Please initial below.
PAYMENT INFORMATION: AUTO draft ACH (Checking or CC) is required UNLESS the term (7w, 14w or 21w) payment is being paid in FULL. A 1 week NON-REFNDABLE deposit is due at the time of registration. The balance of the term selected is due at the star date.  Paper checks will only be accepted at the desk but must be received prior to billing/due date to avoid ACH charge. To process registration payment, please provide bank account ROUTING number and CHECKING ACCOUNT number below along with bank name. ACH Draft is preferred, You will be liable to pay an NSF fee of $25.00 (or the amount allowable by law), which may be automatically debited for each NSF. Credit Cards will be accepted but will have a 3% convenience fee added (we are trying to keep tuition as low as possible.) Please initial that you have read and agree.
