During the school year, your student has had access to a personally assigned Chromebook for use during the school day and at home. When we inventory these devices it is very important that your student has their Chromebook, charger, and case with them at school. Please also complete the form below.

If your student is completing 5th (at K-5 schools), 8th, or 12th grade or if your student will not return to this school next year their assigned Chromebook will be collected. Please be sure to send the Chromebook, charger, and case to school on our inventory day.

If your student’s Chromebook was lost or stolen, please complete the lost/stolen section below. Lost or stolen Chromebooks may incur a replacement fee.

If you would like your student to keep their Chromebook during the summer months, please tick the appropriate box below and provide your signature as permission. Please also review the policies on the next page to ensure that your student and you understand the rights and responsibilities of using a PPS-provided Chromebook
