If you have already registered your child at Fatiha Nursery you will only need to complete your child's details (section 2) and childcare fees (section 6)

If you have NOT already registered your child at Fatiha Nursery you will need to complete all the details for your child.


PLEASE NOTE: We have limited spaces available, so if we are unable to provide a place for your child in this instance we can add them to our waiting list for when a place does become available.
