Formatting Service Request Form
This form is used to express your interest in Jamie Lee Fry's Book Formatting Service. Once your information is submitted, Jamie will review your request and provide you with an estimate for the services you need. If you choose to go ahead with the estimate, you will be asked to submit a sample manuscript and Jamie will prepare two formatting layouts for you to choose from.

Please note that a signed contract and a 50% deposit will be required upfront to initiate the project. The remaining balance will be due upon delivery of the final formatted file(s).

Each format will require its own ISBN number. (Paperback, EBOOK, Etc. . .)

Please note that Amazon's KDP offers free ISBN numbers but you can't use them on other platforms such as IngramSpark.

If you selected Custom Design Headers in the previous question, please note that I will use Standard Licensed photos to create your chapter headers. I do not design the headers from scratch; instead, I may use photo manipulation and combine multiple images to achieve the desired look. Note: I will not use AI images.

Please describe what kind of an image you'd like created. If you don't know, give me a feel for your book and I will come up some ideas for you.

The fee for this service will be anywhere from $10-$30 depending on your answer below.

Also note that chapter header images work best in paperback books. I will include them in your Amazon EPUB (eBook file), but I don't recommend using them on other platforms like Draft2Digital, as they don't transfer well. I will provide a separate eBook file without headers at no additional charge.
