1. I will personally be supervising this activity from start to finish. In the event that I will not be available for any part of this activity, I will notify a member of school administration of my replacement.
2. I will assume liability for any damage or loss of property that occurs during this activity, and hold the Board of Education harmless from claims arising out of the use of school facilities.
3. I understand that the district reserve first claim to the use of its own property, and that cancellations may be issued by administration with or without due notice.
4. I will ensure that this activity is properly chaperoned and that participants are restricted to approved areas(s) of the facility only.
5. I understand that this approval is for the activity/organization listed and I will not assign, transfer, sublet or charge a fee to others for the use of school property.
6. I will be sure to clean all areas used and leave the premises in the condition in which it was found.
7. I will ensure that all participants have vacated the premises and that the facility is secure prior to my departure.
8. I will enforce all district rules during this event and understand that smoking and alcoholic beverages are prohibited on school district property by state law.
