ACKNOWLEDGEMENT:  I have read the USD 231 "Intra-District Transfer Request" procedures.  In addition to completing this form, I am providing all required supplemental material.  I understand if this request is approved by the USD 231 Transfer Committee, the student granted the intra-district transfer shall be admitted on a per-semester basis.  I further recognize this is a voluntary arrangement between the parent/guardian and the school district and shall be reviewed bi-annually (each semester).  The parent or guardian must complete and re-submit to the Clerk of the Board of Education an "Intra-District Transfer Request” form on an annual basis.  The committee will consider availability of space and other factors, including, but not limited to, academic progress, attendance and behavior/discipline records in making a decision on the application.  Lastly, I understand that approval, regardless of “Reason for Request”, is not guaranteed and that USD 231 reserves the right to cancel, revoke, or rescind approved requests at any time. *
