PARENTAL CONSENT FOR AN EDUCATIONAL VISIT

CHALFONT ST GILES JUNIOR SCHOOL – YEAR 4

Year 4 PE Football PE Festival at Misbourne School, Misbourne Drive, Great Missenden, Buckinghamshire HP16 OBN .

On Tuesday 20th September 2022, 12.30 to 15.30 (Event time 13:00 to 15:00)

All of the children in Year 4 will be participating in a Football PE Festival at Misbourne School on Tuesday 20th September 2022 between 12.30 and 15.30 (event time is from 13.00 to 15.00) . The children will be travelling to and from the festival by coach.

As children will leave for Misbourne School at 12.30 , all children will need to bring a packed lunch to eat before they go.

Children should come in to school wearing their school PE kit (green T-shirt, black shorts or black jogging bottoms and green hoodie) and trainers.

Children will also need a named water bottle to take with them.

This PE Festival is organised by the Sports Partnership that is run by local secondary schools. There is no charge for this trip as membership of the Sports Partnership and coach costs to and from DCHS are covered by our Government PE grant.

By completing this form I am giving consent for my child to attend the Year 4 PE Festival at  Misbourne School on Tuesday 20th September 2022. I confirm that I have read the relevant information above. Consent forms to be completed and returned by Tuesday 13th September.

I agree to their participation in the activities described and I acknowledge the need for and expect them to behave responsibly.  I can confirm that the school has current contact and medical details for my child.
