REGISTRATION POLICY: This year NO PAYMENT IS REQUIRED at registration. A confirmation email will be sent to all email addresses provided upon registration. A reminder email will be sent in late June. No payment will be due until we are able to make a decision on whether camp will be in-person. When a decision is made a $50 non-refundable deposit is included in the camp fee of $295. Payments are made online via PayPal. A $45 service charge will be levied on any NSF payment. Band camp runs from 9 a.m. to 4 p.m. Before care is available starting at 8 a.m for a weekly fee of $25. After care is available until 6 p.m. for a weekly fee of $35. Both before and after care can be booked for $50 for the full week. You can book and pay for extended care when making your registration payment or after registering but no later than June 25, 2021. Students do not need any money at camp nor should they bring money or other valuables to camp. *

Session Cancellation: Bandology reserves the right to cancel any session if a minimum number of campers have not registered by June 25, 2021. If this occurs, full refunds will be issued. *

Refunds: Requests for a refund must be submitted in writing no later than June 25, 2021, in order to receive a refund of $245 (does not include the $50 non-refundable deposit). There are no refunds after June 25, 2021. However, we may allow you to switch the registration to another camper – please contact us directly. There are no refunds for campers who do not attend camp, attend only partial days, leave camp early or are asked to leave due to misconduct. *

INSTRUMENT POLICY (for Band Camp): All campers must provide their own instrument, with the exception of percussion players who will use the instruments provided. Campers in Band Camp Junior do NOT need to provide any instruments. We accept no liability for any loss or damage of personal instruments, which should be covered under personal home/contents insurance. If you do not have your own instrument, you will need to borrow or rent an instrument. We have arranged a special Bandology discount with Long and McQuade in Burlington for instrument rentals. Campers must also supply their own mouthpieces, reeds, value oil, etc. There are no supplies on site at camp. *

MEDICAL EMERGENCIES: If your child becomes ill or injured, we will make every attempt to contact you. However, in the event of an accident, injury or illness involving my child, I hereby authorize and grant permission to Bandology staff to secure proper medical treatment and authorize on my child’s behalf all procedures, including, without limitation, admission to an emergency unit, hospital and treatment therein, ordering of x-rays, tests or treatment, injections, anesthesia and/or surgery, as deemed necessary by the attending medical professionals. I agree not to hold Bandology responsible for any costs or injury arising out of an emergency situation. *

CODE OF CONDUCT: The safety of each person is of the utmost importance. Each camper must recognize a personal responsibility to learn and follow at all times the safety and other rules established by Bandology staff. I hereby agree that any behaviour of my child that places herself/himself or others at risk may result in my child’s immediate dismissal from the program. Further, if dismissed from the program, I agree to cover any expenses arising from such dismissal. I hereby acknowledge and agree that no refund will be granted for dismissal or removal of my child before the end of a program session. In order to ensure the safety and well-being of all individuals participating in the program, we reserve the right to alter the program at any time without notice or compensation to the registrant. *

PHOTO/VIDEO RELEASE POLICY: During camp, Bandology may take photographs and/or video recordings of camp activities and events for use as promotion, illustration, art, editorial, advertising, social media, trade, publishing or any other purpose whatsoever. By signing this form you hereby grand permission for photos/videos to be taken of your child and granting us the right to use them for promotion and similar purposes. I hereby assign to Bandology any and all rights, including copyright, financial or other rights, to all photos/videos. I acknowledge that I have no rights financial or otherwise to these photos/videos and grant all ownership and rights to Bandology. I confirm I am over the age of majority in my province, I am competent to execute this form and I have legal signing right for the registered child on this form. *

ASSUMPTION OF RISK AND INDEMNIFYING RELEASE: While Bandology staff, instructors and volunteers will make every reasonable effort to minimize exposure to known risks associated with your child’s participation in our program, I hereby acknowledge that my child may participate in various physical activity or activity that may result in physical injury. In this regard, I agree I have provided a complete and accurate health history and hereby permit the registrant to participate in the full range of activities, except as specifically noted by me in the health history of this registration form. In consideration for the registrant’s opportunity to participate in this program, the receipt and sufficiency of which is hereby acknowledged, I hereby release and forever discharge Bandology Canada Inc. of Oakville, Ontario, its representatives, officers, directors, employees, volunteers and agents, and their respective successors and assigns, from any and all liability for damages sustained in consequence of loss, injury or damage to the registrant, and from all other actions, causes of action, claims, demands or damages of any kind with respect to death, injury, loss or damages to any person or property arising out of, or connected with preparation for, or participation in, the Bandology program. *

As we are located at a public site we cannot guarantee an allergen-free environment. We ask that no nut products are brought in lunches but we cannot be responsible for what other campers pack, or be sure that the equipment, buildings and/ or any item they touch has not come in contact with an allergen.
