I attest and understand that the information below will be released to a third party, Sylvan Learning for the tutorial program.  This authorization will remain in effect from the date submitted until revoked by the me (parent/guardian), in writing, and delivered to the Office of State, Federal and Local Programs, Flint Community Schools, 923 E.
