Event Medical History Form
The Event Medical History form is required for all participants in the 2023 Ninja World Cup in Orlando, FL.  This form is in lieu of a formal physical form from a doctor, although it still can be completed by a medical professional.   

The information submitted will only be accessed when necessary by Ninja World Cup staff.  The data will be held securely and will not be distributed to third parties.   You will be able to change or access this form via the link provided.
