Reopening of School Parent Survey @ Koko Head Elementary
Aloha Parents/Guardians,

Please fill out ONE SURVEY PER STUDENT IN YOUR HOUSEHOLD by Sunday, July 19, 2020.

We are ready to offer our families with two options from the beginning of the school year.  Incoming Kindergarten families will receive a Kindergarten specific orientation schedule next week.  

On this survey we will ask you to select option 1 or option 2 for your child(ren).

OPTION 1 (For Students in PreK & Grades 1-5)
Phase 1: Blended Model
For at least the first two weeks of school (Wed. August 5 - Fri. August 14) all students are assigned to one of two groups (Group A or Group B), by the student's last name.  

Group A (Last Names A-L)
• Face-to-face learning at school (August 5, 7, 11 and 13)
• Distance learning at home (August 6, 10, 12, and 14)

Group B (Last Names M-Z)
• Face-to-face learning at school (August 6, 10, 12, and 14)
• Distance learning at home (August 5, 7, 11 and 13)

A blended model allows us to implement our new protocols and routines and to make necessary adjustments for a safer return for all students every school day as we transition to Phase 2.  Note, the first two weeks of school will be half-days, a detailed schedule for these first two weeks will be issued in a future communication.

Phase 2: Face-to-Face Model
The anticipated start date for this phase will be communicated during the week of July 27
During this phase, the blended model will shift to a “Face-to-Face Model,” which is defined as on campus learning every school day for all students (with the exception of students on the Option 2 track).  We know that most students learn best face-to-face; we are committed to making this happen.

OPTION 2 (For Students Gr. K-5)
A Full Distance Learning (online) option.  Families who select this option must commit to this model for at least one semester.

We understand both options impact your family schedules and child care.  Thank you for making the time to fill out this survey as we plan for a safer return for our entire school `ohana.  Working together, we will make this a meaningful and positive school year for our Cobras.
