I certify that the data given herein and in the accompanying documents are true and correct to the best of my knowledge and thus any misrepresentation is sufficient reason for disqualification. I understand that the student will be accepted on condition in accordance with the regulations of the Department of Education. I also understand that the school reserves the right to dismiss during the school year, any student who consistently fails to comply with its standards of proper behavior, self discipline and academic performance or who is involved in any serious harmful breach of conduct.
