This form will serve as the official sign-up for preseason.  Please make sure to answer each question.
- Location: Littleton High School
-Dates/Times:  Monday-Thursday 4:30-6 PM
- No Attendance Requirement but athletes must be registered for the season with the Athletic Dept. in order to participate
- PAYMENT WILL BE BY CHECK, VENMO OR CASH...venmo: @AndrewFarner, checks made out to Andrew Farner
- Payment will be $60/week OR $150 for all 3 weeks (check, cash, venmo)
- Participants MUST be registered with the Athletic Department to participate in preseason
- Daily Health Questionnaire, Temperature Checks, and Masks are REQUIRED at the beginning of each practice
