Due to the nature of the activities of dance and tumbling there is a risk of physical injury. Regardless of precautions taken to avoid or lessen the occurrences, minor to permanent injuries may occur. By signing my name below, I the parent/legal guardian of the dancer listed above knowingly assume all injury and risks as relating to any dance activity sponsored by Project Move. I knowingly waive any cause of action I might have in tort, negligence, or strict liability against owner, operations, employee, or agent of Project Move. As parent or legal guardian I consent to have the above student participate in activities, classes and programs offered by or at Project Move. Upon signing I agree not to file any claim procedure in connection with any injuries, damages, or losses that may occur, directly or indirectly from participation in activities by or at Project Move. I hereby release owner, operations, employees or agents of Project Move thereof from any liability of any personal injury incurred by myself, chid/ward while participating in any activities conducted by or through Project Move.
