Request to receive the sacrament of Confirmation
By giving your name below, you are requesting to begin the preparations to receive the sacrament of Confirmation. You will be a "candidate" for Confirmation.
Candidate Last Name *
Candidate First Name (formal, the name at your baptism, NOT a nickname) *
Candidate Informal First Name (maybe a nickname, what you are usually called)
Family Email Address *
Family Phone Number of primary household contact (and please indicate if this is a home phone, cell phone, type of cell phone, and whose cell phone. Example: 978-667-3733 Tony Apple Cell Phone)
Candidate Email Address (optional)
Candidate Phone Number (and please indicate if this is a home phone, cell phone, type of cell phone, and whose cell phone. Example: 978-667-3733 Tony Apple Cell Phone) (optional)
Note on candidate email and phone number - minors will always have parents copied on messages sent to them.  Since some families prefer to have their teens included in communication about their activities, we offer the teen email and phone fields as options.
For the next few questions, please answer honestly. Your responses will give us topics to talk about as you make your journey to receiving the sacrament of Confirmation.
Why do you want to receive the sacrament of Confirmation? (sometimes phrased "be confirmed") *
Where do you think your relationship with God is, right now? *
not that close
really close
How would you describe your relationship with God? *
How often do you pray? *
not much, rarely
multiple times a day, every day
What is prayer, to you? *
What is your experience of attending Mass (since Jan. 2022)? Check all that apply. *
Required
Tony's reflections on the Mass: when I was 15-16, I thought Mass was boring! Really, I only started going regularly when I received my driver's license because I thought, if I was going to take Confirmation seriously, I should get serious about whether or not I wanted to be a Catholic. The Mass is rich in meaning and the more I learn, the more meaningful the Mass becomes. But that doesn't mean that you (or I) have to know everything about the Mass now. It's okay if you don't like going to Mass - we can talk through that during our conversation.
When was the last time you experienced the sacrament of Reconciliation? (sometimes called Confession or Penance) (if you don't remember, just say so... it may have been when you were in second grade) *
The following longer-answer questions are best prepared in a separate document, then cut-and-paste your response into this form. Candidates should answer one question, and a parent should answer one question. The questions are below. There are no right or wrong answers. Your responses will give those preparing the candidates for Confirmation a better sense of your family's intentions for the sacrament as well as respecting where the teen candidate is on his/her faith journey.
Candidate Question: please answer one of these two questions: 1. What type of Christian man or woman do you envision being in the future? OR 2. What steps do you need to take to get closer to God? *
Parent's Question: please answer one of these two questions: 1. Have you shared with your teen the importance of this sacrament? OR 2. What is the hope or vision you have for your child after he or she receives this sacrament? *
Questions that I (the candidate) have about the Catholic faith are... (these may be addressed in conversation with Tony)
