This form is for the high school campuses.  If you or  your child does not attend HPHS campus, you have selected the wrong form.  Go back to the main support page and select the correct campus form. Also remember:

-  restarted the device
-  be sure you are using the Chrome browser, even on a Mac computer and your student has logged into Chrome.
