We are excited to be worshipping in the sanctuary again. Please register to let us know which weeks you will attend our Worship Service in person. There is no need to register to watch the service online. We will have an updated registration form each month so that you can register for a month at a time via our website.

Even while we are worshiping inside, we will still broadcast the service at 10:30AM, and you are invited to join us to watch it online via the Sermons page at www.FaithUMC.US or www.OregonCommunityUMC.US (and directly through Facebook if you would like).

We are asking you to answer each of the following questions honestly and completely. This will help us prepare for all of the guests who would like to attend our worship services. You will need to register for each worship service separately so that we have an accurate list of who is attending each week. Please list the name of all of the individuals who will be attending with you from your household.

Providing contact information is vital. That way we can quickly reach out to you if there are changes to worship due to a COVID-19 outbreak or for contact tracing if someone who attends the service later tests positive for COVID-19. Email addresses are key to speed up communication--but we will make phone calls for those who do not have email addresses.

When you arrive at the worship service, we will check you in by family name. If you have pre-registered you will not need to answer any of these questions again, but we will simply ask if you if there are any changes.

If you save the link to this form, you can go in and change your registration if needed. For example, you can go back and add or remove people from your group for the week or make a note if something changes with your health.
