CCPSD employees are working hard to schedule transportation for students for the 2020-2021 school year.

In order to create transportation schedules, it is essential that we have an accurate record of how students are getting to school and home each day.

Please complete 1 survey per child BY FRIDAY, JULY 17th. For example, if you have 3 Carroll County students, you would complete and submit this survey 3 times.

If you are unable to complete this form online, please call your child's school and provide this necessary information.
