The camp will assume no transportation to or from camp. I agree to abide by the disciplines of no smoking, drinking or use of drugs or tobacco on camp premises during my assignment. If accepted, I agree to immediately notify the Camp Director if personal circumstances affect my availability. Please initial as your signature. *

I authorize any references or churches listed in this application to give Camp Hope of East TN any information that they may have regarding my character and fitness for children/teen work. Camp Hope has my permission to do a criminal background check on myself. In consideration of the receipt and evaluation of this application by Camp Hope, I hereby release any individual, church, children’s organization, charity, employer, reference, or any other person or organization, including record custodians, collectively or individually, from any liability for damages of whatever kind or nature which may result at any time to me, my heirs, or family on account of compliance or attempts to comply, with this authorization. I waive any right that I may have to inspect any information provided about me by any person or organization identified by me in this application. Should my application be accepted, I agree to be bound by the bylaws and policies of Camp Hope of East TN and to refrain from unscriptural conduct in the performance of my service on behalf of Camp Hope. I further state that I have carefully read the foregoing release and know the contents thereof and I sign my release as my own free act.
