According to Art. 13 sec. 1 and sec. 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) , hereinafter referred to as GDPR, the Organizers inform that:
  1. Participants' personal data will be processed by the Organizer in accordance with the provisions of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation, "GDPR") for the purpose of conducting the Conference.
  2. Providing your personal data as a participant of the Conference is voluntary, but necessary to achieve the goals of the Conference, as well as necessary to perform public duties related to documenting participation in the Conference, as well as to use his image.
  3. The administrator of personal data is the Academy of Applied Sciences in Siedlce, ul. Sokolowska 161, 08-110 Siedlce. 

I consent to the processing of my personal data provided in the above form by the personal data administrator - the Academy of Applied Sciences Mazovia, ul. Sokołowska 161, in order to complete the registration process and participate in the 2nd International Scientific Conference "Nursing in the Face of Conflicts and Disasters". I have been informed that I have the right to access my data, the possibility of correcting it, and requesting the cessation of its processing.
