PCOCC Incident Reporting Form
This form can be used to report any incidents you may have endured while attending a PCOCC event or in our Discord server. Its intent is to keep PCOCC as safe and inclusive as we can. You can report things anonymously, if you wish, or include contact information for a follow-up.

Please note that PCOCC has limited control over incident reporting outcomes. We can remove and ban people from the club, but do not have the power to administer more serious consequences. Please refer to the links below for alternative Carleton-based reporting mechanisms:
