Please fill this Form out in completion to register your child at Davis School.  Please fill out a separate form for each child that you are registering.  In addition to this form, you will need to submit electronic copies of your child's Birth Certificate, Evidence of Residency, Evidence of Occupancy, Evidence of Identification, and your child's most recent Physical and Immunizations.  Instructions on submitting these electronic documents is listed at the end of this form.
