I am so excited to have your child in my class this year! To ensure that your child is always safe, please fill out the following form to help me make sure that your child is dismissed to the proper location each day. Please note, this is for your child's regular dismissal procedure. If there is ever a change, please contact me via e-mail or written note of the change in dismissal.
