This application should be completed by the group leader or coordinator for any Service with Hope Farm or the Hope Farm grounds, program needs, supervision of children, or students. This is not an employment application. The purpose of this form is to help our volunteer program assist in the creation of a safe environment for children or students who participate in the programs of Hope Farm or use Hope Farm facilities.
