I hereby give my approval for my child’s participation in any and all activities of Colorado Sting Basketball Club LLC, Shepherd of the Hills Lutheran Church, Rocky Mountain Christian Church, and Alexander Dawson School LLC during the selected session. In exchange for the acceptance of said child’s candidacy by Colorado Sting Basketball Club LLC, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Colorado Sting Basketball Club LCC, Shepherd of the Hills Lutheran Church, Rocky Mountain Christian Church, Alexander Dawson School LLC, and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of tryouts, travel to, participating in, or returning from practices, exhibitions, parades or competitions conducted during the season.

There is a risk of being injured that is inherent in all sports activities, including basketball. Some of these injuries include, but are not limited to, the risk of sprains, fractures, paralysis, or death.

In case of injury to said child, I hereby waive all claims against Colorado Sting Basketball Club LLC, Shepherd of the Hills Lutheran Church, Rocky Mountain Church, and Alexander Dawson School LLC including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event.

Furthermore, all images and videos of Colorado Sting Basketball Club LLC are the property of Colorado Sting Basketball Club LLC and can be used for any promotional consideration.

I hereby state that I have carefully read the above waiver. Acceptance and understanding of this agreement are hereby acknowledged.
