Player Level - For multiple children in the same family, the most expensive level is full price, 2nd most expensive level is 1/2 price, and 3rd child is $50.  A $75 payment must accompany this registration form to secure the rates below.  $25 of your registration is non-refundable.  All returned checks will be charged a bank fee of $35.  If your check is returned, all future payments must be cash only.  If hockey is shut down due to COVID-19 executive orders, registration will be refunded on a pro-rated basis.

I agree to pay the registration balance due by 12/10/2020.  I understand failure to pay registration by 12/10/2020 and/or failure to pay wreath/pizza kit money or complete any other fundraising requirements will result in my child(ren) not skating with North Country Youth Hockey. IP/Mini-Mite Level (sell a combination of 10 items).  Mite through Bantam level (sell a combination of 20 items).
