1) PLEASE READ THESE DIRECTIONS: To be eligible for this scholarship, you must be interested in pursuing a career in the field of education such as: teaching (any grade level, Pre-K, college), school counseling, speech therapy, school physical therapy, Career and Technical education majors, media coordinators, school IT specialists, etc.
To apply for the local chapter scholarship (approx. $800 offered each year, but it may be more depending on the chapter's fundraising successes), PLEASE complete the following required fields in this Google Form. THEN, see the bottom of this Google Form for additional requirements you must submit. Please mail, fax, or email this additional requirement (transcript) to the contact person named below. Transcript must arrive to the contact person (named below) via email or mail by Friday, March 15 (recommended mailing date for letters of recommendation and other required elements is Monday, March 11) . This Google Form must be completed by Friday, March 15, 2024 at midnight to be eligible for the scholarship offered.

This form must be completed at the time of submission. You will not be able to save your data and return back to it later!
