Orders must be placed before November 30. After you submit this form you will receive a copy of your responses via email. That's why your email address is needed.

The wreaths cost $35 each. This is a sample of 2021 wreaths. The 2022 wreaths will be similar. Since we assemble our wreaths by hand, no two are exactly alike.

Delivery Options

Pick up at Ludmila's house.
Delivery to your home.
Pick up at church office.
Pick up during a class.
Pick up at the DSC Christmas Market. (Sunday, December 4, at the church lounge.)
When would you like to have them delivered?

All wreaths will be ready during a three week period, each on Tuesday, Wednesday, Thursday or Friday. Please select the best week.

December 4 at the DSC Christmas Market

If you are not picking up during a class, no response is needed.

If you want home delivery, please include your address. Otherwise, this is not needed. Delivery is available only in the Corvallis, Albany, Philomath, Adair area.
