2024 UHF Scholarship Application
Before completing this application, please be aware of the following requirements:
Applicants must be Utah residents.
Scholarship applicants must have a diagnosed bleeding disorder, be a carrier, or be the parent of a child with a bleeding disorder.
Scholarships will be awarded for undergraduate studies only.
Applicants must be enrolled in a college, university, trade, or technical school to be considered.
All scholarship awards must be used by the end of the calendar year in which they are issued.
Along with this application,
you must submit two letters of recommendation and your most recent transcript
from the last school you attended (see below for more information).
Incomplete applications will not be considered.
Application must be submitted by May 31, 2024.
This deadline is final.
