The Wellness Center is now offering COVID-19 Testing for our patients. Please fill out the following information for an appointment request. We will respond with a testing time and date. Results can be expected in 24-48 hours and will be reviewed via a TeleMedicine call with our Medical Director, Dr. Mira Kaga. Please arrive 10 minutes before your appointment time to the address above with your ID, Insurance card, and the following form ready.
Testing will be run through LabCorp. By signing this form you are acknowledging that LabCorp will be billing your insurance company for the cost of testing. If you have insurance, the test should be at no cost to you. If you do not have insurance, the out of pocket cost for testing is about $50 and will be billed directly to you by LabCorp.
