ZKSwap ( V1 ) Withdrawal Demand Registration Form
With ZKSpace launching,   ZKSwap (V1) will completely shut down at 7:00 UTC on December 31, 2021 and relevant features was closed too.

We urge users to migrate all funds to ZKSpace asap. Left-behind idle funds are safe on-chain after the shut down of ZKSwap (V1) network but users will have to contact community moderator and fill in this form for asset withdrawal.  

Notice:
1. Users need to fill in the following form first, and we will re-open the ZKSwap (V1) service at a specific period for users ' withdrawal. The specific time is subject to the ZKSpace official notice.
2.When the ZKSwap (V1) service is reopened, we will notify you by email or @user in the telegram group.
