The Student Policy Handbook is available to view on our school website (www.bacards.org). A separate copy of the 2020-21 COVID ADDENDUM to the Parent/Student Handbook was included in our back to school mailing. Please review this with your child/ren. Note that each student must complete this form. Copies are also available in the school office. Take careful note of the Technology Guideline policy that is included in the Student Handbook. The school's COVID Response Plans are also posted on the school website under the COVID Response Plans tab. This form is to be electronically signed by both student and parent/guardian.

Consent to Release of Student Photos and Videos During Online Learning: The Family Educational Rights and Privacy Act (FERPA) establishes certain rights for parents regarding the privacy of their child’s educational record. Access to or release of the educational record is only by written parental consent. (Please check each box for agreement) *

I hereby consent to release of personally identifiable information or education records about my child including, but not limited to, photographs, video, or audio disclosed as part of online or virtual education undertaken during a period of the COVID-19 pandemic. Note: A request to withdraw this consent must be submitted in writing to the school, dated and signed by me.

I also understand that the school is required to the release of personally identifiable information to appropriate public health agencies of any known or suspected COVID-19 diagnosis pertaining to my child.
