This form contains General Waivers and Activity Waivers that must be signed before use of any facilities.

You CAN NOT sign on behalf of persons not in your immediate family.
Each immediate family must sign the waiver independently.

You may also sign for the minors or adults for whom you are the Legal Guardian. There will be a section to indicate their names and acknowledge that you are responsible for explaining the risks to them, and therefore take full liability upon yourself and you agree to all risks, terms, & conditions in relation to their activities.
