This form is used to give as much information as you possibly can in regards to your cover. Please make sure you fill this in more than 24 hours before your booking date. PRETTY IN INK CREATIONS POLICY OUTLINE: 

• 50% of the invoice MUST be paid upon receipt of the invoice
• PIIC does NOT offer any refunds
• PIIC does NOT offer payment plans
• Custom Packages must be paid IN FULL before any files are sent out 
• Design process can take anywhere from 1 - 3 days 
• You get 2 free revisions, after that my hourly rate will apply of $25 an hour
• ABSOLUTELY NO MICRO MANAGING OF ANY KIND IS ALLOWED OR YOU FOREFIT THE POLICY WHICH CAN CAUSE A PAUSE/CANCELLATION IN YOUR DESIGN
