TMA Device Request Form
Dear TMA family.  We hope that you are managing well in the current situation.  As we transition to the use of remote learning, TMA will provide loans of devices for scholars to use.  Please complete this survey to provide us information to use to best support you with a loan of a device. (PLEASE COMPLETE THIS SURVEY BY NO LATER THAN FRIDAY, MARCH 20TH).

Determinations of loans will be based on need.

Distribution of devices will be done on Monday, March 23rd from 10 am - 1 pm and Thursday, March 26th from 10 am - 1 pm. You will be contacted to schedule a time for item pick-up.
