Boys Major Showcase (the "Event")

The novel coronavirus ("COVID-19") has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

Hockey Night in Boston (“HNIB”) has put in place preventative measures to reduce the spread of COVID-19; however, HNIB cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending the Event could increase your risk and your child(ren)’s risk of contracting COVID-19.

By completing and submitting this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and/or I may be exposed to or infected by COVID-19 by attending the Event and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the Event may result from the actions, omissions, or negligence of myself and others, including, but not limited to, HNIB and its employees, contractors, agents, representatives, vendors, volunteers, and other Event participants and their families.

In consideration of my child(ren) being allowed to participate in the Event during the COVID-19 pandemic and for other good and valuable consideration, receipt of which is hereby acknowledged, on behalf of myself, my child(ren) and all others who may claim by, under, or through me, I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my and/or my child(ren)’s attendance at and participation in the Event. I do hereby agree to indemnify and hold harmless and do hereby release, acquit, covenant not to sue, and forever discharge HNIB and all of its employees, agents, representatives, contractors, vendors, Event volunteers and assigns, and all other persons or companies from any and all claims, actions, or causes of action which I or my child(ren) now have, or which may hereafter accrue, whether for personal injury or property damage, whether known or unknown, arising out of or in any way resulting from my and/or my child(ren)’s participation in the Event during the COVID-19 pandemic, including, without limitation, the summer months of 2020 (“Claims”).

I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of HNIB, its employees, agents, representatives, contractors, vendors and Event volunteers whether a COVID-19 infection occurs before, during, or after participation in the Event.
I understand and agree that my completion and submission of the form below represents a signature on behalf of myself and each of my children.
