DOG ADOPTION APPLICATION - SUNNY DAYS RESCUE & REHAB
Thankyou for expressing interest in adopting one of our very deserving doggos! This is a no obligation adoption application designed to assist us in matching your home and lifestyle with the dog you're applying for, or another dog that may be more suitable. You are not obliged to adopt a dog by completing this form, it is just a step in the process should you choose to go ahead with an adoption.

Any personal information submitted in this form is not used for any purpose other than keeping a record of who has adopted our animals - if you do not end up adopting, your information will be removed from our system after a short period.

Thankyou :)

Adoption Terms & Conditions

Returns & Refunds:
- If you adopt a dog from us and decide to return it within your adoption trial period of 4 weeks, and we deem no harm has come to the dog, you will receive a full refund. If we believe however that the pet has been psychologically or physically negatively affected in your care during the trial period or after, you may receive part of or no refund.
- If you decide to return your adopted dog to us after the adoption trial period of 4 weeks has completed, regardless of the reason, you forfeit any refund of your adoption fee.

Medical Costs:
- During your adoption trial period, if your adopted dog presents with a relapse of a pre-existing illness or injury that we should have known about, disclosed to you and treated, or an illness arises that may have been present or incubating while the dog was in our care prior to adoption, we will cover medical costs *providing you contact us prior to taking the dog to a vet clinic* so that we can arrange, where possible, for them to be seen by one of our partnering vet clinics, for cost reasons, as vets are not obligated to provide discounts to rescue organisations and many don't. Without the support of vets who partner with rescue organisations, rehoming animals would simply not be possible. It
- For new injuries or illnesses which were not present while in our care, we are not responsible for costs either during or after your adoption trial period. However we may be able to assist with a discounted rate for treatment if we use one of our partnering vets, please contact us if you need this assistance.
- For 'Foster With View to Adopt' arrangements the same conditions apply as for a normal adoption trial period in that we will cover medical expenses for 4 weeks in certain circumstances- please see the first point under 'Medical Costs' above.

Harm to this pet:
If we become aware that the physical or psychological welfare of this pet is at risk in your care, during or after your adoption trial period, we may seize the pet without notice and seek police support to do so.  If after consideration we decide the situation to be resolved and the pet faces no further threat, it will be returned to you.  However if we feel the situation cannot be resolved we will begin sourcing a new home for the pet and you will receive no refund for their adoption or care whilst being with you.

Lifetime Guarantee:
We offer a lifetime guarantee to welcome your pet back into our care if at any point you’re no longer able to care for it. Please contact us if you find yourself considering rehoming this pet; we ask that you please don't give the animal away, sell it, or pass it on to a family member as more often than not these scenarios don't work out for the animal.

AGREEMENT OF THE ABOVE TERMS
By adopting this pet you agree to our adoption terms and conditions during, and after, your adoption trial period which starts the date your pet goes to your home and ends on the last day of the fourth week thereafter.

Specifically you agree:
1. To contact us immediately should this pet become lost or you have any concerns for it's welfare.
2. To provide for this pet appropriate food, fresh water, hygiene and an enclosed, safe area at night.
3. To provide regular vet treatment for this pet, including but not limited to vaccinations, parasite control and immediate treatment of illness or injury.
4. To adopt this pet in it's current state knowing that financial costs may arise in future for medical or behavioural conditions.

6. To keep your contact details up to date on your pet's microchip listing.

8. That you will not surrender, give away or sell this pet to another party, including a rescue group, shelter, veterinarian or individual, and that you will contact us to arrange for the pet to be returned to us so we can continue it’s care and rehoming.

Do you agree to keep your adopted dog on lead in all situations until you're aware of its temperament and what may trigger a change in behaviour?
Dogs behave differently in different situations and its not always clear when first adopted. We highly recommend taking appropriate measures and time to get to know your dog's behaviour. We can suggest a fantastic trainer who won't cost a bunch and will be able to give you a report on your dog's behaviour across all scenarios - please ask.
