This is NOT a commitment or a registration for virtual learning
The purpose of this form is to gain an understanding of how many families may choose the virtual option if we return to school under the Face to Face Model (Phase 4-5). Based on what we know now (July 15, 2020), Phase 4 and 5 will include a normal five days a week schedule with full days of instruction.
We will ask families to officially register for the virtual option by August 6, 2020 via another communication. We understand every family will make a decision based on the safety and well-being of their child. Our goal is to provide a quality education to our students and provide support for our families through both a Face to Face Learning Model and a Virtual Learning Model.
Completing this form is not a commitment to virtual learning but rather a mechanism to help us develop a robust plan for students not returning Face to Face this fall.
