Camp Phoenix 2023 Pre-Registration Form/Camper Information
PLEASE READ BEFORE CONTINUING: 

This form DOES NOT guarantee admission into camp for the 2023 season. The information provided here will only be activated as an official registration once registration and weekly payment(s) are received. Payments can only be made at the camp site on pre-registration/open house dates or during camp hours and should NOT be mailed. (Cash or check only) Payment must be received no later than the Thursday of the week prior to attendance. There will be NO EXCEPTIONS. Campers will not be allowed on site without payment having been received. (Updated camp fees are listed on the front page of our website and also at the end of this form.)

Payments are non-refundable.

For each child's pre-registration, a separate form must be completed.
